Ethics approval and consent to participate
: The LV-KOS project was approved by the Danish Data Protection Agency (J.no. 2009-41-4069) and the Danish Health and Medicines Authority (J.no. 7–604–04-2/122/EHE). According to Danish law, approval from the regional Committee on Health Research Ethics was not needed for this study as no specific biomedical intervention was performed besides ordinary clinical examinations and treatments. We had oral consent from the GPs and written consent from parents to participate. Thus, the study did not present any ethical problem or dilemma.
