Ethics approval and consent to participate
: This study was approved by the Institutional Ethical Review Board of the Faculty of Medicine Ramathibodi Hospital, Mahidol University (ID 07–58-59). All patient information was anonymized and deidentified prior to data analysis. The consent obtained from the participants were verbal and the completion of our survey was considered implied consent to participate. The IRB approved this method of obtaining consent, because our subjects are members of a community in which signing forms is not the cultural norm, and our research presents no more than minimal risk of harm to subjects as it involves no procedures requiring consent outside the context of participation in our surveys.
