Ethics approval and consent to participate
: All data was handled in accordance with the rules of the Danish Data Protection Agency, and participant anonymity was maintained throughout the study. According to Danish law, approval from a Medical Ethics Committee was not required for this feasibility study. All patients and GPs gave informed consent to participate in the study. JS had only telephone contact with the participants (except from the first info meetings with the GPs in the health centres), and the consents were therefore obtained verbally. This was not specifically approved by an IRB.
