Ethics approval and consent to participate
: This paper is the result of a Quality Improvement study conducted via an online survey and did not require formal ethics approval as per the 2014 Tri-Council Policy Statement: Ethical Conduct for Research Involving Humans, which states in Article 2.5 that quality improvement studies do not constitute research for the purposes of the Policy and not fall within the scope of a research ethics review board in Canada [CitationRef removed]. Survey respondents were informed that by answering the written online survey they would be providing consent to participate. Therefore, those who responded the online survey were considered to have consented to participate in the study.
