: Since this study was not a medical-scientific investigation and since no experiments were done on human subjects, ethical approval was waived by the Medical Ethics Review Committee at Erasmus MC, University Medical Centre Rotterdam (MEC-2018–1628). Written informed consent was obtained from all participants at the beginning of each interview and will be stored for a maximum of 15 years in a protected area at Erasmus MC.
