: Approval from the Regional Committee for Medical and Health Research Ethics was deemed unnecessary according to national regulations [CitationRef removed]. The project is not due to submission for ethical inspection, following the Act of Medical and Health Research Law, article §10, cf. research ethics article §4, Sect. 2 (48). Informed consent to participate from all informants was obtained and they signed a written letter of consent. A compensation of NOK 1000 was given to all informants.
