Ethics approval and consent to participate
: The protocol was approved by the Conjoint Health Research Ethics Board (CHREB) at the University of Calgary (REB# 14-479), Calgary, Alberta, Canada. A signed consent form was completed by participating site implementation team members only, as they participated in multiple data collection activities. For all other groups that completed surveys, implied consent was obtained. Consent was waived by the CHREB for patient enrollment in early mobilization and retrospective review of related administrative data, given consent was not feasible under the Alberta Health Information Act.
