: After giving written informed consent in accordance with the International Ethical Guidelines for Biomedical Research Involving Human Subjects (CIOMS) 1993, participants were interviewed face-to-face by trained personnel using computer-assisted questionnaires. Moreover, a sound medical examination including anamnestic interviews, blood samples, ECG, blood pressure measurement and many other examinations was conducted. The study was approved by the institutional review boards of the Ethical Commission of the Medical Faculty of the University of Duisburg-Essen and a quality management according to standard industrial norms (DIN EN ISO 9001: 2000) was implemented by TÜV Med Rheinland.
