Ethics approval and consent to participate

: Approval for this study was obtained from the medical ethical commission of the Erasmus Medical Centre of Rotterdam (MEC 2015–748). Respondents in the internet survey were asked for written consent for participation and publication. Professionals and key informants who were interviewed or who took part in focus group discussions were asked verbal consent for participation and publication. The ethical committee requested written consent for questionnaires of patients only.
