: This study was a secondary data analysis using published data, with the exception of the use of Secondary Uses Service (SUS) data. The SUS data were analysed by FG in the Health Foundation secure data environment, which is accredited to the NHS Digital Data Security and Protection Toolkit and the ISO27001 Information Security Management standard. Summary statistics were shared with co-authors only after statistical disclosure control. SUS data are routinely collected, retrospective and pseudonymised data made available through an agreement approved by NHS England with the Health Foundation. Therefore, consent to participate nor ethics approval was required for this study.
