Declarations

: Ethics approval was waived by “the Ministry of Health and Population Institutional Review Board” (IRB) as a non-research study using data from public health surveillance that only involves routine data collection and procedures carried out pursuant to the Ministry of Health and Population at the Ministry hospitals.
: The study used data collected during patients’ routine management procedures. All patients verbally consented to participate after they were fully informed about the aims of surveillance and the risks associated with participation. The verbal consent was approved by The Ministry of Health and Population (MoH) in Egypt. Patients’ private information is coded so investigators cannot readily determine the identities of those to whom it pertains.
