: All subjects involved in our study were extracted from the French REIN registry which received agreement from two Ethics committees. Ethical approval references were the CNIL (Commission Nationale de l’Information et des Libertés) with agreement number 903, 188 and the CCTIRS (Comité Consultatif sur le Traitement de l’Information en matière de Recherche dans le domaine de la Santé) with agreement number 03.149. Verbal informed consent to participate was obtained from all subjects involved.
