: Both studies reported in this manuscript were performed in accordance with the Declaration of Helsinki. The studies were approved by the Ethics Committee of Helsinki University Hospital. In the first study i.e., “the kinetic cohort” (ethics committee reference number 144/13/03/02/2013) written informed consent was obtained from all patients before enrolment to the study. In the second study i.e. “the FINNAKI cohort” (ethics committee reference number 18/13/03/02/2010§124), deferred consent policy was approved and the informed consent was obtained either from patients or their next of kin prior to the use of gathered data.
