: The survey was conducted as a market research survey adhering to the ICC/ESOMAR International code on market and social research [CitationRef removed] and the European Pharmaceutical Marketing Research Association guidelines [CitationRef removed], and in full accordance with relevant legislation at the time of data collection including the US Health Insurance Portability and Accountability Act 1996 [CitationRef removed] and Health Information Technology for Economic and Clinical Health Act legislation [CitationRef removed]. Therefore, ethics committee approval was not required. Prior to volunteering to complete a questionnaire, all patients were asked to provide written informed consent. The survey instructions describe the purpose of the survey, why the respondent had been selected and who might have access to the aggregated and anonymized dataset compiled.
