Ethics approval and consent to participate
: The trial was approved by West of Scotland Research Ethics Committee, reference number 15/WS/0129. This Ethics committee had the earliest available date for ethics review and was chosen for this reason as per frequent UK practice. The approval covered all sites in the trial: management (“Research and Development”) approval was also sought and obtained through all NHS organisations prior to recruitment from those organisations, in full accordance with UK research governance requirements. Fully informed consent was obtained from all participants, with parents providing proxy consent for infants.
