: We confirm that all methods were carried out in accordance to relevant guidelines and regulations. We used data from the MR CLEAN registry and from the DASA audit. The MR CLEAN registry was approved by the ethics committee of the Erasmus University MC, Rotterdam, the Netherlands (MEC-2014–235). With this approval, it was also approved by the research board of each participating center. At University Medical Center Utrecht, approval to participate in the study has been obtained from their own research board and ethics committee.
