Ethics approval and consent to participate
: Ethics approval for this study was obtained from the Biomedical Research Ethics Committee at the University of KwaZulu-Natal (BE301/15) and the KZN Department of Health. All participants signed a consent form before voluntarily participating in the study. For participants who were younger than 18 years, written consent was obtained from a parent or legal guardian and assent from the participant. To preserve anonymity, codes were assigned to each participant based on the area, type of participant and number of the visit. Audio recordings and transcripts were stored at the University of KZN in a password protected file.
