Ethics approval and consent to participate
: This study has been conducted in accordance with the guidelines of the Declaration of Helsinki and Good Clinical Practice. The patient data were retrospectively acquired from hospital records generated during care-as-usual. Statistical analysis was performed requiring patient anonymity in agreement with the ethics committee regulations [CitationRef removed]. According to the Dutch law on Medical Research with Human Subjects, this study has been exempted for approval by the local ethics committees and no informed consent was deemed necessary. The ethical committees of the University Medical Center Groningen and Isala Klinieken Zwolle reviewed the research and also decided that no approval is needed.
