Ethics approval and consent to participate
: Ethical approval was obtained from the University of Zambia Biomedical Research Ethics Committee (UNZABREC), the National Health and Research Authority (NHRA) of Zambia, and the University of Alabama at Birmingham (USA). Additional permission was granted by the Lusaka District Health Office to conduct the study at the selected facilities. Written consent was obtained from all participants prior to data collection. Confidentiality was maintained by anonymising all data forms. Guardians of participants below the age of 18 were required to give consent and participants to assent before they could be included in the study.
