Ethics approval and consent to participate
: Ethical approval was granted by the Ethics Committee of R.D. Gardi Medical College, Ujjain, India (119/2010), the Indian Institute of Public Health, Gandhinagar, Gujarat, India (23/2012) and Karolinska Institutet (2010/1671–31/5). The study was described to all study participants by the research assistants. Written informed consent was obtained from the participants before they were enrolled in the study and responded to the questionnaire. For illiterate participants, a literate family member or neighbour was requested to verify the consent form. Upon approval, a thumb print was taken from the participant to acknowledge consent.
