Ethics approval and consent to participate
: Ethical approvals for both the NUHDSS and the Urbanization, Poverty and Health Dynamics research project were granted from the Ethical Review Board of the Kenya Medical Research Institute. All the MCH field workers were trained in research ethics and all the respondents gave written informed consent before the interviews. There were 237 women who were under the legal age of consent (below the age of 18) at the time of interview. The respondents who were the heads of their households or living with their husbands were considered as emancipated and provided direct consent, and all others who lived with parents or guardians gave assent and the parents/guardians gave parental consent.
