Ethics approval and consent to participate
: The present study was approved by an Institutional Review Board: The Regional Ethical Committee in Uppsala, Sweden (approval number 2014/504).All participants received written information about the aim of the study, that participation was anonymous and completely voluntary, and that they could decide to discontinue their participation without a need for explaining why. The name and contact details of the principal investigator, i.e., the last author (including address, email and telephone number) was also provided. Participants were informed that the results of the study would be published in a scientific journal and that individual authentic quotes would be used in the article. Information was given that only the researchers involved in the study have access to the data. Consent to participate was given by clicking <i>yes</i> in a box associated with the following question: “Do you consent to participate in the study?”. Thereafter, the respondents were given access to the questions in the survey.
: The participants were informed that the results of the study will be published in scientific journals and that individual authentic quotes may be used in the articles. Consent to participate was given before the respondents had access to the questions in the survey.
