Ethics approval and consent to participate
: Ethical approvals were obtained at national and regional levels from the three countries in which data were collected. In Ethiopia, national ethical approval was obtained from the Ministry of Science and Technology of the Federal Democratic Republic of Ethiopia, and regional ethical approvals were obtained from the Amhara Regional Health Bureau, the Oromia Regional Health Bureau and the Tigray Regional Health Bureau. In Uttar Pradesh, ethical approval was obtained from the Ethical Review Board of the Society for the Promotion of Ethical Clinical Trials (SPECT-ERB), and from the National Rural Health Mission in Uttar Pradesh. In Nigeria, national ethical approval was obtained from the National Health Research Ethics Committee of the Federal Ministry of Health in Abuja, and state approval was obtained from the Gombe State Ministry of Health in Gombe. Ethical approval for the surveys was also obtained from the London School of Hygiene and Tropical Medicine (reference 6008).All survey respondents provided informed, voluntary written consent to be interviewed.
