Ethics approval and consent to participate
: Ethical approval was obtained from Kilimanjaro Christian Medical University College (KCMUCo) Research Ethics Committee (CREC no. 901). Permission to conduct the study was first sought from the Regional Medical Officer and Regional RCH Coordinator of Mbeya region. After receiving the permission letter from the RMO, the respective DMOs, District Reproductive and Child Health Coordinators (DRCHCo) and District Executive Directors (DEDs) were visited and informed about the survey and its aims. The DRCHCo helped with the introduction to ward and village and/or street leaders in selected enumeration areas. Written informed consent was obtained from each participant and in case of illiterate women a right thumbprint was used. Where participants are children (under 16 years old) written informed consent for participation in the study was obtained from their parent or guardian. Privacy and confidentiality were maintained by using only unique identifying numbers in the questionnaires. The questionnaires were kept in the locked cabinet throughout the time in the field and data entered was stored in password protected computer.
