: Ethical approval was obtained from the ethics committee, Tanzanian National Health Research Ethics Review Committee (NatHREC), of the Tanzanian National Institute for Medical Research (NIMR/HQ/R.8c/Vol. I/1950). Letters of permission for the study were obtained from the Ministry of Health (Tanzania), the management of the Dar es salaam regional health authorities and the management of the participating health facilities. The study is a retrospective review of medical records. All patient identities were anonymized and assigned a research number. The authors confirm that during this study no patients were interviewed, nor were any experiments performed on humans or human tissue samples. Individually obtained informed consent was therefore deemed not required. A waiver for informed consent was obtained from the ethics review committee (NatHREC) of the Tanzanian National Institute for Medical Research.
