: The study protocol, topic guides and semi-structured interview schedules gained national NHS ethics approval by NRES Committee South West - Cornwall & Plymouth (15/SW/0260) and in accordance with the Governance Arrangements for Research Ethics Committees and complies fully with the Standard Operating Procedures for Research Ethics Committees in the UK.
