: The Surgical Ethics Committee of Helsinki University Hospital (Dnro HUS 226/E6/06, extension TMK02 §66 17.4.2013) and the National Supervisory Authority of Welfare and Health (Valvira Dnro 10,041/06.01.03.01/2012) approved the study. Helsinki University Hospital approved our retrospective collection of data and specimens (Dnro HUS 269/2017). For this type of retrospective study, written consent from participants is according to the Finnish law not required.
