: This study was approved by the National Cancer Center Institutional Review Board (the reference number: 2016–455) and by the Institutional Review Boards of Shizuoka Cancer Center, Kanagawa Cancer Center, National Hospital Organization Osaka National Hospital, Kobe University Hospital, Fukuyama City Hospital, National Hospital Organization Kyusyu Cancer Center, and Chiba Cancer Center. Obtaining informed consent was not necessarily required by Ethical Guidelines for Medical and Health Research Involving Human Subjects because the present study was utilizing existing information retained by the research implementing entity and did not utilize human biological specimens. Access to the clinical/personal patient data used in the present study was implemented according to the Ethical Guidelines for Medical and Health Research Involving Human Subjects by the permission of the National Cancer Center Institutional Review Board and the IRBs of the other 7 participating institutions.
