: <i>Survey data</i>: The study was approved by the Norwegian Data Inspectorate and the Regional Committee for Medical and Health Research Ethics (S-04255). The women were informed about the study in a cover letter, and the ethical approval granted that answering the questionnaire constituted informed consent to participate in the study. All methods were carried out in accordance with relevant guidelines and regulations.<i>Registry data</i>: Cancer and screening registration is mandatory and does not require consent. Registry data analyses were performed with anonymous data. Access to anonymous registry data was granted to address specific research aims and was approved by the University Hospital of Oslo privacy officer (17/10342).
