: Ethical approval was obtained from the Mbeya Medical Research and Ethics review Committee, the Tanzanian National Health Research Ethics Committee, and the Ethics Committee of the University Munich (LMU). All methods were carried out in accordance with relevenat guidelines and regulations. The study had been fully explained and answers were responded before written informed consent was obtained from the participant. In case of illiterate study participant a thumb print was provided by the participant and the form had to be signed by an impartial witness. This informed consent method, including for illiterate study participants, was approved by all involved ethic comittees: the local Mbeya Medical Research and Ethics review Committee, the Tanzanian National Health Research Ethics Commitee and the ethics committee of the Ludwig-Maximilian-University in Munich, Germany.
