Ethics approval and consent to participate
: This study was assessed by the ‘Quality Improvement Team’ board of NHS Lothian, which determines if formal institutional ethical approval is required. The board deemed that formal institutional ethics approval was not necessary for this survey. Completing the questionnaire and returning it was interpreted as consent to participate. All participants were informed prior to completing the questionnaire that their participation was entirely voluntary, that by completing and returning the questionnaire they were giving their consent to participate in the study, and that their individual responses may be published. No patients were involved as part of this survey.
