: The study was carried out in line with the Guidelines of the Helsinki Declaration for human studies (as amended). Ethical approval was obtained from the Health Research Ethics Committee of University of Nigeria Teaching Hospital, Enugu (NHREC/05/01/2008B-FWA00002458-1RB00002323). Informed written consent was obtained from each patient before participating in the study. Consent was sought from parents/guardians for children. They were made to understand that participation was free and voluntary withdrawal from the exercise was permissible at any stage of the study. Withdrawal attracted no prejudice on their management. Data used for the study did not include specific patient identifiers and data analysis was done at aggregate level so that results could not be traced to individual patients.
