: Health Research Authority (HRA) approval – HRA-IRAS reference 159209.University of Exeter Medical School Ethics Committee (REC Application Number: 15/07/070).For all participants, written consent was documented prior to the interview and all young people were offered a £10 voucher as an incentive. Informed consent was gained from all participants aged 16 years and above. For participants under 16, their assent and the consent of a person with Parental Responsibility (as defined by the Children Act 1989) was gained.
