: The study was approved by the ethics committee of the Ludwig Maximilian University, in Munich, Germany, under the reference number 350–14, and written informed consent was obtained from all participants after participants have read the information material. Protocol modifications (amendments) are reported to the ethics committee of the Ludwig Maximilian University prior to implementation.
