: The study was approved by the institutional review board of the participating hospitals, including the Research Ethics Committee of the National Taiwan University Hospital (NTUH-REC no. 200705030 M) and the Ethical Review Committee of Taoyuan Mental Hospital, Department of Health, Executive Yaun (C200705021), and has therefore been performed in accordance with the ethical standards laid down in the 1964 Declaration of Helsinki.
