: All adult participants gave written, informed consent for participation in the study, parents/legal guardians gave consent for the child. Children were always informed about the possibility to decline specific tasks for any reason.The study was approved by the Danish Data Protection Agency. The Danish Ministry of Health granted permission to retrieve data from the Danish registers. The study protocol was sent to the National Commiittee on Health Research Ethics, Capital Region of Denmark, Hillerød, who decided that ethical approval was not needed due to the observational nature of the study (protocol number H-4–2012-045).
