Ethics approval and consent to participate
: The Youth Health Survey received ethics approval from the Health Research Ethics Boards and the Cooperative Acitivites Program (CAP) at the University of Alberta. The authors received approval from the Health Research Ethics Boards of the University of Alberta, Canada and the Cooperative Activities Program (CAP) to collect and use the data for the Youth Health Survey. Parents provided written consent for their ward’s participation and students provided verbal assent to participate in the Youth Health Survey.
