Ethics approval and consent to participate
: The study has obtained ethical approval from the Regional Ethical Review Board at the University of Gothenburg, Sweden, with the reference number 234-16. Informed written consent was obtained from all participant PHCC managers, participants in the focus group interviews, and participant patients. Written and oral information stressed that participation was voluntary and could be stopped without consequences. Data are kept and handled according to safety standards. Analyses are done at the group level, and results are presented so that individuals cannot be recognized.
