Ethics approval and consent to participate
: This study was approved by the Addis Ababa University School of Public Health Research Ethics Committee. Letter of permission was also obtained from North Showa Health Office and respective health offices in the three selected districts and kebeles. Consent forms were attached to every questionnaire with brief information regarding the main aim of the study, benefits, and risks of the study to the participants, explaining their full right to withdraw at any time and to, skip any question that they don’t want to answer. Written consent was sought from each study participant. Those who consented, participated in the study. Data was collected at household level in an area which gave maximum privacy, after 40 days of locally accepted bereavement period. We removed an individual identifier to maintain the anonymity of the respondents by assigning each questionnaire a unique number before submitting to causes of death assignment. All of the data was collected from the parents or from the guardians in cases where the parents had died.
