Ethics approval and consent to participate
: The study was reviewed and approved by the Research Ethics and Committee (REC) of AKU, Kenya. The committee is accredited by the National Commission for Science, Technology and Innovation (ExternalRef removed). Written consent was obtained from each participant for both the quantitative and qualitative part of the study. Parents/legal guardians of participants under the age of 16 gave written consent for their child to participate in this study. We offered no compensation for participation in the study. Audio recordings of FGDs and KIIs were transcribed, translated, and de-identified.
