Ethics approval and consent to participate
: In this research article, secondary data that were made available to us by the Uganda Bureau of Statistics and the DHS Program (ExternalRef removed) were used. Ethical approval was obtained by the above bodies from the Institutional Review Board of International Consulting Firm of Calverton, Maryland, USA, and from Makerere University School of Biomedical Sciences Higher Degrees Research and Ethics committee and the Uganda National Council for Science and Technology. Information on ethical approval is published in the 2006, 2011 DHS [CitationRef removed, CitationRef removed] and 2009 MIS reports [CitationRef removed].Consent to participate was obtained by the Uganda Bureau of Statistics and the DHS Program from the Institutional Review Board of International Consulting Firm of Calverton, Maryland, USA, and from Makerere University School of Biomedical Sciences Higher Degrees Research and Ethics committee and the Uganda National Council for Science and Technology. Interviewers read an informed consent statement to the respondents, and an interview was conducted only if respondents assented verbally.
