Ethics approval and consent to participate
: The Lake Zone Institutional Review Board of the National Institute of Medical Research (No. MR/53/100/353) and the Institutional Review Board (no. 14674–001) and the Animal Care and Use Committee (protocol no. 04577–001) of Washington State University approved this study. Village leaders granted permission for the study to be carried out locally. In accordance with the existing MDA program and the requirements of the implementing District Medical Office (DMO), verbal consent to treat primary school aged children was given by the Head Teacher of each school. If the child was not attending school an attending guardian gave verbal consent. Children attending clinics alone were not given treatment. All instances of verbal consent were recorded in writing in the Treatment Register by the DMO Nurse implementing the MDA. In accordance with the existing MDRV program and the requirements of the implementing District Veterinary Office, verbal consent to vaccinate dogs was given by owners. All instances of verbal consent were recorded in writing in the Vaccination Register by the veterinarian implementing the MDRV. In accordance with the approval from the Lake Zone Institutional Review Board of the National Institute of Medical Research and the Institutional Review Board of Washington State University, written consent, or assent for children, was obtained from all interview participants.
