: The study was approved by the Research Review Committee (RRC) and Ethical Review Committee (ERC), the two obligatory components of the institutional review board (IRB) of the icddr,b. During data collection, informed written consent was taken from parents on behalf of children. As for the adolescents, written informed consent/assent consent was obtained from their parent or guardian. The respondents were told that their participation was voluntary and they had the right to withdraw at any time during the interview.
