: This research was submitted to the Johns Hopkins Bloomberg School of Public Health Institutional Review Board and deemed to be “non-human subjects research.” Additional approvals were obtained from: Institutional Review Board of the Ministry of Public Health, Afghanistan, the James P. Grant School of Public Health Ethical Review Committee, the Kinshasa School of Public Health Institutional Review Board, the Institutional Review Board of the College of Health Sciences of Addis Ababa University, the Institutional Review Board for Protection of Human Subjects, IIHMR, the Medical and Health Research Ethics Committee, Gadjah Mada University, and the National Human Research Ethics Committee, Nigeria.
