Ethics approval and consent to participate
: The medical Ethics Committee of the University of Antwerp studied the study proposal and draft manuscript and deemed ethical advice unnecessary. The approval from a medical Ethics Committee was not mandatory according to national regulations (2004/22376/BE, 7 May 2004 and the update 2014/18155/BE of 4 April 2014) [CitationRef removed] and the guidance document about non-interventional studies ([CitationRef removed], page 38). Participants to the surveys were not subject to any actions or rules of conduct, and no personal or medical data was processed. Participation was anonymous. Participants were informed about the objective and design of the study and their rights, and provided written informed consent before participation to the survey.
