Ethics approval and consent to participate
: Scientific and ethical approval was obtained from the World Health Organization Ethical Review Committee (protocol ID, B40121) and the Kilimanjaro Christian Medical College Research Ethics and Review Committee. All participants received written informed consents and before the interview and they were allowed to withdraw from the study at any time without penalty. If participants could not read and write, the information sheets and consent forms were read out to them in presence of an impartial witness and a thumbprint was taken.
