: Data used in this study were pseudonymised primary care data extracted from general practices who are members of the RCGP RSC network. These data were extracted into a secure network. Patients who opt out of data sharing are excluded from the extraction process.This study was defined as a study of “Usual Practice,” by the UK Health Research Authority decision tool (ExternalRef removed) and as such did not require formal ethical approval. The study was approved by the RCGP Data Access Committee.
