Ethics approval and consent to participate
: All procedures performed in this study, involving human participants, complied with the 1964 Helsinki declaration. In Poland there is no requirement for ethics committee approval for studies similar to ours [CitationRef removed]. Nevertheless, at the beginning of the study its purpose was explained to the respondents who were assured that they would not be identified in any presentation or publications. Participants were included in the study after obtaining written signed informed consent. They were assured about confidentiality, and that their participation would be on a voluntary basis, as well as that they had full rights to withdraw from the study at any time. To protect the confidentiality of the subjects, completed questionnaires were stored in a locked filing cabinet. Computer data were password protected and accessible only to the study investigators.
