Ethics approval and consent to participate
: The methodology of the study was approved by the ethics committee of the James P. Grant School of Public Health, BRAC University, Bangladesh and Spatial Sciences faculty of Groningen University, the Netherlands. At the field level, written informed consent was obtained from the participants before the start of the interview or the group discussion. Most of the participants could not read the consent form, but were able to sign it. These participants had someone close to them read the consent form to them before they signed it. To protect the anonymity of the information provided and the identities of the participants, the personal data of the participants, such as their name and household address, were not entered into the transcripts. The participants were given the flexibility to refuse to answer questions or to withdraw their participation at any time if they felt uncomfortable.
