: Ethical review and approval for this study were obtained from the Sierra Leone Ministry of Health and Sanitation Ethics and Scientific Review Committee as well as the Institutional Review Board of the Harvard T. H. Chan School of Public Health (Protocol #15145, Approval #17). All participants in the survey were over the age of 18 and enrolled following informed consent delivered by a team of 16 trained local staff. All methods were carried out in accordance with relevant guidelines and regulations. Participants completed verbal informed consent, due to low levels of literacy in the sample as well as ongoing outbreak conditions during data collection in the two study districts which prohibited the exchange of physical research materials between participants and research assistants. Verbal informed consent procedures were approved by the both the Sierra Leone Ministry of Health and Sanitation Ethics and Scientific Review Committee and the Institutional Review Board of the Harvard T. H. Chan School of Public Health (Protocol #15145, Approval #17).
